48px avatar image missing for user

One of the authors for my blog doesn’t have a 48px avatar created. There’s a 32px one used when she comments but if I change the Recent Comments widget to show 48px avatars or if she shows up in the Authors widget, the 48px avatar does not exist.

Is this a new avatar? I see the 48px one. It usually takes some time for avatars to propagate through the server structure until they are globally available.
Try to see if the avatar appears after flushing your cache.

It’s not a new avatar. I see ‘File not found’. I already flushed my cache and cleared all cookies after the dashboard upgrade.
Why would the 16, 32, 96 and 128 pixel avatars exist for someone but not the 48?

Avatars can be a little unstable in the first twelve hours after uploading. I’ve noticed occasionally a former avatar of mine will show up, but that hasn’t happened in awhile. I am able to see the avatar in that link, though. Perhaps you have a cookie issue? Clear your browser cache and cookies and see if you can see it normally.
It could be a server issue on WordPress’s side, but if that’s the case we can expect a lot more reports to come soon.

I already flushed my cache and cleared all cookies after the dashboard upgrade. Should I repeat this every time a new author joins my blog?

Well, the answer to my question is “yes” because the avatar picture shows up in another browser. *sigh*
