  Hi there,
  I accidentally deleted the HTML code for the Contact Form in my Blog (‘Button’ Theme) can anybody please help me by sending me the code again?

  You can create a new contact form quite easily, here is more information about it
  https://en.support.wordpress.com/forms/contact-form/

  It has the shortcodes as well to help you with it.
