add an entry in the registration form

  Hello,

  I would like to know if there is a plugin that allows you to add an entry to the registration page for example the type of blood (A, B, O) of the person,
  also if a person is vaccinated etc,

  and then to be able to have a statistic of this information on the administration panel.

  Yes, you can customize the contact form to include required fields. I’m not sure on the logistics of inquiring about such information and whether or not that would be hipaa protected. We can’t provide legal advice but it’s something for you to consider.
