Add Checkbox Input To WordPress Comment Reply Form

Hi there,
Any suggestions for how to add two checkboxes after a comment (before submit reply), one for visitor to allow site owner to publish comment, and another to allow site owner to publish the visitors name?
And a way for site manager to view this in the backend?

If you are referring to a page http://en.support.wordpress.com/pages/ that has a contact form set up, and you mean contact form feedback then know it’s for private communication that does not appear on the blog. http://en.support.wordpress.com/contact-form/ -
I know it’s not possible in wordpress, but does anyone know any other forums that discuss how to do this manually?

You cannot add two fields to the comments form on any WordPress.COM blog and no matter what solutions are posted on other forums they will not apply to WordPress.COM hosted blogs.

Thanks for replying – I see the issue – I ‘m referring to a different wordpress site, do you know of any solutions for that?

I’m sorry but if that site is not a WordPress.COM hosted blog then we cannot help you, as we do not provide support for any blogs that are not hosted by WordPress.COM.
WordPress.COM and WordPress.ORG are completely separate and have different logins, features, run different versions of some themes with the same names, and have separate support forums. http://en.support.wordpress.com/com-vs-org/
If you don’t have a username account at WordPress.ORG click http://wordpress.org/support/ and register one on the top right hand corner of the page that opens, so you can post to the support forums there.
