Add Custom form not working

    Good Day…I am a newbie at this, so if I don’t ask the right question I am sorry. I want to use the “add custom form”, but it is not working. I used it before and created a drop down menu in my U17 & U14 pre registration on line form. I closed it and now I want to open it again. I went in today to change it around, but it is not working.
