Add fields to post form

  I have used WordPress to create several sites for my clients. Some of them are approaching content management systems. However, I have one client, a physician, that wants to change the structure of the post form. Instead of just Title and then a text box for the post content, he wants to have more control.

  Example boxes:

  Case Title
  Case summary
  Findings
  Discussion

  I found one plugin that can add boxes to the end of the form, but not at the beginning.

  Any other plugins or ideas?

  (PS– I host these blogs on my own server, so I can make changes)

  That really isn’t possible here at wordpress.com. We cannot upload plugins. I expect where you want to be is over at http://wordpress.org/support/ where the stand-alone, self-hosted wordpress software support is.
