Add Media isn't Adding Media

i’m doing what i’ve always done to add a picture into a post… i’m in… ‘not visual mode’ mode.
1: put the cursor where i want to insert the image.
2: click the “upload/insert” icon.
3: fill out the form to upload my image, give title, caption, center, full size, no link.then when i hit the ‘Insert into Post’ button… the Add Media popup window goes blank and hangs. and doesn’t insert the code into my post. i can close the popup window, but… no code.
when i go back and do it again, i can find the image on the Gallery tab of the Add Media popup. it just won’t get into my post today. is there something screwy going on with this today? did something go wrong? (i wont accuse anyone of ‘breaking’ anything, like what happened with the ‘copy a post’ thing. :X) is this a new bug i need to report or a temporary glitch?

Please answer the following so we can eliminate some possible causes of your issue.

BETA Chrome versions are causing lots of issue because BETA = buggy. So if you are running Cjrome make sure you are running the latest stable version.
5.Have you tried using the browser with all extensions or add-ons disabled?

SOLUTION: (at least one of them)
Do not hit ENTER to create line breaks in the caption portion of the form. Instead, use the HTML break symbol ‘
‘.I will report this as a bug; it should at least throw an error or something to make it obvious what is wrong.

Just in case anyone doubted, this solution has worked for me. I had exactly the same, erratic problem, and removing the hard returns from the caption, replacing them with the BR tag solved it.
