Add order form to my page

Hi, I need to add an order form to a page and I followed the instructions in http://en.support.wordpress.com/contact-form/. After clicking on “Add this form to my post” and also clicking “Preview Changes”, I do not see the form on the page at all. The page only shows a big box field for Comments. Am I missing some steps? Many thanks to anyone who can help.

In the page editor page, click on the “screen options” tab at upper right and activate the discussion module if it is not already active. Then down below the text area, click the right end of the discussion module title bar and disallow comments and trackbacks and then update the post.
I’m seeing your contact/order form just fine so the above will finish you up.
