Adding 3rd level to drop downs

  Hi there,
  on http://www.applecrossingsvc.com/request-service/ I would like to add a 3rd level to my drop down menus, but I am unfamiliar with editing at this level. I also want to verify the email correctly. Can anyone help point me in the right direction?
