adding a capture form with video on a new page

  I’ve seen on some sites that have a page for example “generate leads” and when you click on the tab it automatically goes to their video capture page. How is this done?

  The blog I need help with is: (visible only to logged in users)

  Hi, can you give us a link to a page where that is done so we can take a look?

  There are two options that come to mind right away that are available here at WordPress.com. One would be the subscription widget which can be placed in the sidebar. The second would be a contact form. The first would save the captures for you in your dashboard. The second would require you to manually add the email addresses to your email program or email/newsletter service.

  Go to http://www.richardmatharoo.com and click on the generate leads tab for an example. Thanks for responding so quickly.

  You can use a contact form for that, and you just put whatever video and text you want above the contact form code. When someone fills out the contact form, their information will be sent to your email address.
