Adding a 'complete the fields' form on a page

I am trying to add a form on a page, a form that has more fields (boxes) where people can submit more details – such as name, work, address etc. How would I do that? Can I do that in the basic format or do I need to upgrade?

Looks like you can use this internet application Wufoo to create a form and then embed the resulting shortcode — so it’ll be possible with the free version of WP.

Thank you so much! I used Wufoo and so far it seems to work. I also looked into the Add Contact Form Button but my editor, assumingly the newest version, doesn’t seem to have that option any more, though it would come in very handy. I really appreciate your support, Kulash!
Any suggestions on how to add a logo next to the Header/Title of the blog?

You should be able to go My Site –> Customize –> Site Logo to add/fix your site’s logo, title, and tagline.
