Adding a form. Not collection info

  I created another form on my Peaceful Dove page and the address does NOT fill in. You can type the address in but when you send it. No information comes through. How can I correct that ?

  The blog I need help with is: (visible only to logged in users)

  Please post an active link to the page staring with http:// you created this form on. Note also that the only form we can use on free hosted WordPress.com blogs is the contact form > http://en.support.wordpress.com/contact-form/
