adding a long complicated form- with clearly formatted results

  I wanted to add a plugin but using Sela, it looks like you have to use the contact form- no choice.

  I am editing the form code, but cannot format so the display is satisfactory.

  And when i email the results, the content is really difficult to decipher. As an intake form for a medical practice, the way the content is being returned is not legible or useful.

  What would you suggest
