Adding a question box to a page

  I would like to add a place on my page where it says, “Do you have a question?” It would also need a place that they could type in a question and some sort of connectivity so that I could be alerted that a question had been asked. How do I do this?

  The blog I need help with is: (visible only to logged in users)

  Add a contact form

  http://en.support.wordpress.com/contact-form/

  then add the link to that contact form page in the MENU or on the sides, and label it Do you have a question?
