How do I add a “Choose File” link to my form page? Is there a plug in for this?

It doesn’t look like the Forms block includes that functionality. You will likely need to utilize a plugin.
There are a few plugins that include “Choose File” functionality.
Forminator, WPForms, and Jotform.

The Form block doesn’t support uploading files to forms.
You’d need to use a plugin like, for example, Contact Form 7, which supports that feature.

Hello @ortizworks There is such an option with Jotform, Wufoo and Google that don’t require a plugin. (NAYY) If you can’t embed the form here on your WordPress.com website, you should be able to link it to their site.
