adding Contact Form giving me trouble

  I recently transferred my blog from a self-hosted site over to WordPress.com and the Contact Form is now showing up merely as [Contact-Us-Form], so I deleted it.

  So then I tried to add the Contact Form by clicking on the button that is offered ‘Add Contact Form’, but although I can see the sample, it won’t let me add it via the button that says, ‘Add this form to blog.’

  I keep clicking and nothing happens.
