Adding survey/poll and linking results to a file upload

  I appreciate all help in advance. Thanks all.

  I am conducting a research study where participants need to upload a file + complete an optional survey questionnaire. It is important that the file and survey results are linked in some way on the back end.

  The page itself can be quite simple. Some text explaining the research, perhaps an image, the survey, the upload.

  I’m wondering if my wordpress.com site could accomplish this (even if I need another theme) or whether I should build this out using a custom wordpress.org website using plugins.

  Any guidance would be appreciated.

  You can do this with surveys from our sister company, Polldaddy. You can sign into Polldaddy with your WordPress.com account.

  You can then embed your survey into your site, using the WordPress.com shortcode:
