It looks like you are using your form in a widget. There are quite a few entries on the forums about changing the width of the form. There is this one about using the form in a widget although I don’t know if it is theme specific:

I tried the first suggestion and it is still cut off on the right side of the form. Any suggestions?

Well the second link provides a lot more similar responses that you could read and try out those that seem to apply, but if you have the customization, you can wait for a knowledgeable staffer with the right testing environment to give you the answer. I am just a dabbling fellow user who likes to experiment. Good luck.
