'Apply' Button not working

  Hi, all.

  So I’m fairly new-ish to WordPress – And I usually overcome each issue that I come across.

  Basically, if you scroll down to the ‘Apply’ section, you will see a Apply button. This used to bring up a form with what details are required, although now it just shows a group of boxes.

  No idea how this has happened, how would I amend this form to show which details are required?

  Are you seeing something different? If so, can you send over a screenshot of what you are seeing? Then I’ll be able to take a closer look.
