There is no application for an unused blog URL process. That’s because there is no expiration date on .wordpress.com blogs unused, reserved or otherwise. If you can make contact on your own with the owner of an unused blog via submitting a contact form on the site or commenting on a post on the site, then providing the site owner is agreeable to it there is a means of a consenting blog owner to transfer the blog to another username account. http://en.support.wordpress.com/moving-a-blog/#transferring-your-blog-to-another-user-or-account

In the case that there is no contact information or you do not get a reponse to a contact form submission or a comment on the site, Staff will not breach the privacy policy and provide you with an email address.

If you are not successful, you can see here for changing a blog address https://en.support.wordpress.com/changing-blog-address/ or here to register another blog with a similar name. http://en.support.wordpress.com/register-a-blog/

Or you can register a domain via a domain mapping upgrade and it will not matter what the underlying .wordpress.com address is.

I believe it could be my old account but i forgot my password or username to that account.

When we register a username account the matching blog is reserved for our future use. It cannot be obtained by anyone other than the person who has registered the matching username account.

If and only if you have registered the matching username account then log in under the correct username account and as the blog is reserved for you and you will register it after logging in under the correct username account.
    https://signup.wordpress.com/signup/?user=1

However, if you changed your username then the blog URL will not be available for use under the new username account.

Every WordPress.COM blog has a sole owner. It is the person with the WordPress.COM username account who registered the blog under that username account and a single associated email address, and who is the original Admin of the blogs registered under that username account.

You have to be logged in as Admin http://en.support.wordpress.com/user-roles/#administrator under the exact same username account that registered the blog to access the blog’s dashboard and delete the site. https://en.support.wordpress.com/delete-site/ or make it private. To change blog visibility to Private go to > Settings > Reading scroll to Blog Visibility and choose option 3. See the guide here http://en.support.wordpress.com/settings/privacy-settings Also note that blog URL deletion is permanent and irreversible. https://en.support.wordpress.com/recycling-blog-names/

Clicking this link should display all blogs registered under the same username, including the hidden ones: https://dashboard.wordpress.com/wp-admin/index.php?page=my-blogs&show=hidden

If you do not see the blog here http://en.support.wordpress.com/my-blogs/ then
    follow this guide > http://en.support.wordpress.com/my-blogs/#my-blogs-dashboard-visibility

If the log-in information is lost visit https://en.wordpress.com/wp-login.php?action=lostpassword

If required please complete the form for account recovery at this link https://en.wordpress.com/wp-login.php?action=recovery and submit it to the Account Recovery Staff so they can contact you by email ie. not on these public support forums which are available to everyone with internet access.
