Are legitimate comments being filtered as spam?

I am having trouble with spam and the general directions for dealing with it are not helping.
I received the following comment. He keeps getting the same post over and over after asking to be notified. THIS IS NOT A GOOD THING! He’s not getting new posts and now wants to be removed. Meanwhile, I get ridiculous amounts of Uggs boots and other ad spam — even after I blacklist them. I am beginning to think that I am missing legitimate comments. “After I originally commented I clicked the -Notify me when new articles are added- checkbox now when a comment is added I recieve four emails sticking with the same comment. Can there be any method for you to remove me from that service? Thanks!”
HOW CAN I ENSURE THE INTEGRITY OF COMMENTS, i.e. legitimate comments come through and spam not? I have managed my settings per your directions and there seems to be a glitch. Akismet says it’s caught 531 spam comments. How do I ensure that it’s really spam? I need help with this. I don’t want to miss legitmate comments and keep getting crap (although very little real crap comes through) Thank you for your help with this.
