Attaching a form to a button on Call To Action Page

  On my call to action page, there is a button already developed on the page. I want to link that to a form so people can subscribe to my blog via that button. How can I do this?

  The blog I need help with is: (visible only to logged in users)

  There are 2 versions of WordPress. What happens here at WordPress.com is unrelated to using the open source WordPress installation which is what you have set up now.
