  Please let me know how to add attachment tab in contact form

  Attachment field types are not available currently on WordPress.COM because there is no FTP access to upload files.

  Here are the list of support field types:
  • Checkbox: allows the user to tick a box to make a choice
  • Dropdown: creates a dropdown of options
  • Email Address: requires an email address format
  • Name: text field
  • Radio Button: similar to the checkbox field type
  • Text: a single line of text
  • Text Area: several lines of text
  • Web Address: requires a URL format

  So what you can do instead is use a simple text field and ask users to provide a link to the file.
