Best practice for developing new themes.

Hi, I’ve created a couple of WordPress blogs with months in between each project. So, I have some experience but I’m always having to refresh my memory of how I did things previously.
I’d like to ask those of you who work with WordPress all the time what is the best way to develop. Specifically I have the following questions:
1. Where do you host a project while you’re developing – until it’s been finalised and approved for going live?
The reason I ask that is because in the past I’ve hosted the project on my own hosting account while developing it and then when finished, transferred it to my client’s hosting account. Seems this could be an avoidable step. I’m thinking maybe, I should host the site with WordPress and point my clients domain name to that location. Is that a good idea?
2. I’d like to develop a theme from scratch but, although I have good experience with CSS and HTML, I’m really new to PHP. And since there are so many great themes available it seems like a waste of time to develop something if something already exists.
I’ve seen some really amazing themes and I’d like to have other features like roll-over image effects for navigation (using the sliding door technique rather than swapping images) and I’d also like to be able to add jquery galleries. Is that easier done by building your own theme from scratch or can add these features to existing themes?
That’s all for now but there will be lots more because this year I’m really going to get in to WordPress. I love it!

If you’re not familiar with the process of developing new themes specifically for WP.com, I suggest you contact staff and ask. It’s not simple.
