Hi there,
Your issue looks like a plugin issue. If you have FTP access to your site, try to manually disable all plugins at the /wp-content/ folder.
Here’s a guide to show you how: https://www.wpbeginner.com/plugins/how-to-deactivate-all-plugins-when-not-able-to-access-wp-admin/
After you have gained access to your site, you can consider installing Health Check plugin to help in troubleshooting issues on your site before you re-activate the plugins.
If the steps above do not help, try to contact the Mad Mini Sign Up Forms plugin support here: https://wordpress.org/support/plugin/mad-mimi-sign-up-forms
Since your site is a self-hosted site, you’re recommended to post your questions at the WordPress.org forum and seek help from the experts and volunteer members there. You may need to register an account if you don’t already have one.

Please be informed that the staff and members of this forum could only help you out with support topics related to WordPress.com sites but not self-hosted WordPress sites as they’re separate entities and independent from each other.

Hope this helps.
