Broke my site trying to add a mailchimp popup

I was trying to add a Mailchimp exit popup to my site… http://donavantwebsites.com/
I selected and copied the mailchimp code, and navigated to my site…I think I might have somehow edited the php file. If you look at the site there’s not a huge black space near the top of the pages with
