Can I insert custom fields in the comment section?

  I would like to have custom comment fields on certain pages of my blog. For instance, I list information about performers on some pages, and on those pages I would like to have a comment form including the fields: Year, Library, Cost, and Comments. Please let me know if this is possible, otherwise I’ll just insert comment instructions at the bottom of all these performer pages, thanks!
