Sandbox is a practically blank sheet, a theme- where people with good css skills can modify and style the built-in css selectors.

You see, the brilliant thing about css is that you can call things whatever you like. That means that all the diffrent themes can have diffrent selector names for example “main content”. It might be called “maincontent” in one theme, and something else in another. Unless the selector names are the same in the css-script and the html, it will not work.
You can play with the css as a preview, but if you want to publish those changes you make, you need to pay for the css upgrade.
Don’t pay until you’ve understood css. There are lots of good tutorials around on the net.

For permanent, published changes, yes.
But do not pay until you’ve made the changes work in the preview, and are positive that it works as you want it to. -

It’s an annual fee. If you’re interested, do look into the tutorials mentioned upthread because there’s no official support for CSS here.
