Cannot add a contact me form to a post

Can’t find the add contact form button.
According to the tutorial there are three buttons All on the same row and the last one is add form
But I see only two buttons:
Add media
Add poll
Below them there is a small custom form button but it doesn’t let me do much.
Help! I want to post today and I’m stuck!The blog I need help with is: (visible only to logged in users)

What’s the issue? You say you see the contact form button. That’s it. What, specifically, do you want to do that it doesn’t let you do?

There is a third icon (camera, circle, form)

Thank you both for responding so quickly. The problem must have been with my laptop permissions. I tried again with my iPad and got it to work.

I am having the same problem – the contact form button, which i have used many times before, is now gone. Help!!?
