Cannot add MailChimp Subsriber pop up form

  I followed instructions according to WordPress.com on how to add MailChimp’s subscriber pop up form, which includes the fact that I can’t connect my site to MailChimp. But MailChimp does not allow me to publish my form unless i connect my website to mail chimp, and the code provided is the code for integration with MailChimp instead of code for the form. How can I still add the MailChimp form??
