can't cancel and refund theme upgrade

When i try to the cancel and refund window that comes up has nothing but a title. no way to proceed. no place to say reason for decision. I canceled a different upgrade successfully.
The blog I need help with is: (visible only to logged in users)

Refunds are time limited. WordPress.com provides a 30-day refund on all upgrades except Domain Registrations, Domain Renewals, and Guided Transfers.
Note: It takes from 1 – 2 weeks for the refund to be received.

http://en.support.wordpress.com/Billing-History/Provided you are logged in under the same username account that purchased the upgrade, and you are within the allowed time frame for refunds, you can cancel and request a refund from your dashboard > Store > My Upgrades. OK, that’s not a problem! I have cancelled that theme and have refunded you for that purchase. Depending on your bank and their policies you should be seeing the funds back in your account in a few days to a few weeks.
