Can't embed video- no problems previously

  I can’t seem to embed video, starting today-no problem previously. The upload/insert icons look different- If I recall correctly, there was a photo icon, a video icon, and a custom form icon. Now I see custom form, photo/video together, and a poll icon. I’ve clicked on the media icon, attempted to embed video, but failed. Please help!
