Where is the "add contact form" media button?

  Support instructions say to click “add contact form” media button and I do not have one where the picture indicates it should be.

  This is because your site is a self-hosted installation of WordPress.org. WordPress.org and WordPress.com are actually separate entities.

  For your WordPress.org site you could install the Grunion plugin to get the same custom form options.
