can’t find Form in the block

  When I create a page in nmoiss I can find the form in the block, but when I try to add a form to a page in my other site I can’t find it.

  WP.com: Yes
  Jetpack: Yes
  Correct account: Yes

  The blog I need help with is: (visible only to logged in users)
