Can’t get my Woo Premium theme on WP

Hi guys! I need some help with getting my premium theme to wp. I have made many attempts that I can’t seem to figure out what I’m doing wrong and I’m in a time crunch with nothing resolved yet. Can someone help me?
The other problem I came across was my opt-in form. Can’t seem to get it on the side.

Are you asking about the site linked to your name?
If so, then the answer is that you can use only the themes available under
Appearance—> Themes.Or are you asking about a self-hosted blog?
