Can’t remove name of person commenting on contact form

  Once I was able to establish a Contact Form on my new blog on my contact page, I had someone send in a comment to test it and now I cannot seem to remove the name and e-mail address from my contact form. It shows up everytime I open my Blog. How can I clear this out of there?
