Capture Email for Subscription

On my blog I would like to capture an email address and zip code and provide a button for the user to select where I can persist the data entered in our site’s database. Can you tell me if there is a widget I can use? If I use just a text area, can I put additional code behind that text area to do what I need?

What’s available to your here for your free hosted WordPress.com blog is to enable blog subscriptions on your blog and place a blog subscription widget in your sidebar for those who wish to subscribe and receive your posts by email.
Clicking on the active number will show you the users who have subscribed with our Subscription feature in the admin bar. I have the subscription widget on my blog, but when those click “subscribe,” they get an e-mail notifying them that they have subscribed, but there e-mail addresses do not always get added to my subscriptions list. This has happened several times since I started the blog (I’ve watched friends subscribe and receive a confirmation e-mail, but there subscription doesn’t appear on my end), any suggestions?
