  How do I chance the name ‘Home’ of the menu item

  Create a custom menu and include a Page in it that has the replacement word you want for “home” in it.

  Create Page http://en.support.wordpress.com/pages/#create-a-new-page
  Create custom menu http://en.support.wordpress.com/menus/
  Add the Page you created to it http://en.support.wordpress.com/menus/#adding-pages and add all other Pages and items you want to appear in the custom menu bar.
