So my blog.url is johnsoutofoffice.wordpress.com but it goes to gothamgrind.com. How do I get rid of gothamgrind.com and get it to go johnsoutofoffice.wordpress.com

You must be logged in as Admin http://en.support.wordpress.com/user-roles/#administrator under the exact same username account that registered the blog and purchased the upgrade to access the blog’s dashboard. Then see this page on how to cancel a domain: http://en.support.wordpress.com/domains/cancel-domain/#cancel-domain

I found how to do it now but do I have to pay another $18? Why can I not just unregister one and switch it to the new name?

The domain gothangrind.com is registered under the ‘gothamgrind’ user account. The domain can’t be switched since it’s already been registered since 2013-04-04, and won’t expire until 2015-04-04.
However, you can have as many additional domain names linked to one blog.
