Changing opt-in html for one page changes others as well

  My blog has 2 pages and 2 posts. I want each of them to have different opt-in box info. But when I enter specific html into the widget box for page 1, it alters page 2 as well. Same for the posts.

  How can I get my Aweber web form html/java script specific to one page or post to register only on that page/post, without affecting the others?
