Checklist/Form with Authentication

Hi all, new to wordpress.
Trying to create a checklist/form for my organization.
The process is as follows:
1) Employee completes and submits checklist/form
2) Supervisor gets notified of submission. He reviews and allow/reject submissionDepending on the result, employee’s screen will show if he is allowed to continue work or not.
Is there a form plugin that can put the submission process on hold until further actions by the supervisor? What are my options?

Hi there,
We don’t have any specific functionality like that by default. We do offer a way to set up a submission form, which emails the results to any email address you wish.
However, that form is not tied into any other functions, so it does not have the way to block a user or prevent them from proceeding in some way.
To clarify, you are trying to give folks a way to sign up to your site, but you are witholding access until they complete the form? I’m not sure what you meant by “allowed to continue to work or not”
My guess here is that you may be able to add this kind of functionality via a “plugin” (which will require our Business Plan) but with a bit more background info we can see if there is an off-the-shelf plugin that will allow you to add that functionality to your site.

Think of the form as a safety checklist. The employee is only permitted to continue with work, say, drive a high risk vehicle, if the supervisor has reviewed the form and grant permission. Otherwise, if certain fields in the form hits a no-go criteria, for eg wet weather conditions, then the supervisor will select the no-go option in his version of the form and the employee will be notified that he is not allowed to drive the vehicle.

You would need to use a form solution that utilises conditional logic. These types of form solutions usually come in plugin form.
