Collecting descriptive information from commenters

    In addition to name and email address I would like to use check boxes to collect information from commenters so I can later collate their responses by the kind of person who has commented (e.g., trail hiker, bicycler, motorcyclist, etc.).

    – AND –

    At the same time make each person’s comments visible to the other blog visitors.

    I know about WuFoo and it works fine but a person’s comments are sent off to a database at WuFoo and are not available for subsequent blog visitors to read.

    So, how do I collect a bit of descriptive information about my blog visitors while making the comments everyone makes visible for all to read?
