Comment FORM doesn't display

  Still it doesn’t appear in my blog.
  I tried to change theme but still after changing theme comment form doesn’t appear.

  Comment form should suppose to look like this as per my theme http://m.uploadedit.com/ba3s/1489505221521.jpg
