Comment template has my details in it

  When I add a comments template and save it the template on the pages has my details in the boxes that are designed for readers to leave their details. How do I remove my details?

  The blog I need help with is: (visible only to logged in users)

  Hi there,

  Are you speaking about this:

  

  Please note that when someone is logged into their WordPress.com account, it automatically fills in that part of the form. So, since I’m logged into my account, you will see in the screenshot that it’s actually my name and email address on your contact form. Since you are logged into your account, it is probably showing you your details.

  When I log out of WordPress.com it is actually empty as seen here:
