comments are appearing out of order

  New comments sometimes appear out of sequence. Instead of a recent new comment (not a “reply” to an earlier one) appearing at the bottom it can sometimes appear way up in the middle of a thread.

  Example: http://vridar.wordpress.com/2012/07/10/reply-to-hoffmanns-on-not-explaining-born-of-a-woman/#comment-33092 — This appears at #12 even though it was only posted after and later than comment #23.

  What is going on? What can I do to ensure the most recent comments appear at the bottom of the thread?
