Comments keep going to moderation for one reader

My blog settings: I approve first-time comments only.
However I have one reader whose comments keep going into moderation despite having been approved more than once. This is despite using the same email address to log in every time she comments. That same email address was used to comment on a different blog without any issues. I don’t understand why it’s different on my blog.
I think the only new thing is that she logs in with Gravatar now to comment. My blacklist is empty.The blog I need help with is: (visible only to logged in users)

Our software redacts ie. hides email addresses so only Staff can read them. Please type that email address here in the conventional format for email addresses. Hi there,
This is really something we’d be better able to troubleshoot with the user in question, as the issue is likely with their account or the way they comment rather than with your site.
For example, do you know if that person is already logged into their account when they try to comment on your site, or do they log in via the comment form when they try to comment? What are the exact steps they follow when commenting? What browser do they use? Does it make a difference if they use a different browser? Do they have third party cookies enabled in their browser?
That same email address was used to comment on a different blog without any issues.
Can you give me the address of the other site? And is there currently one of this user’s comments in moderation on your site so I can check it? If there is, on which post?

I was waiting for that user to comment and she has. Once again, it went into moderation. I have no idea whether she logs in first or at the comment box itself. I just see that she has a Gravatar account.
The other site where she used to comment without issues is here. User name E–e.
She did not appear to use a Gravatar account then.Her most recent comment on my blog is on this post. The comment is still in moderation.

I notice that commenter has different usernames showing on the comments. This makes me think they might be commenting while already logged in one time, and commenting without logging in, just providing a username and email address the next. Our system is likely pushing the comment into moderation because of the mismatch of the name to the email address in that case.
If the commenter makes sure they’re always logged into their WordPress.com account before attempting to comment, this should stop happening.
