Comments stuck in moderation queue even though "Akismet cleared this comment"

Which commenters on your site are being sent to moderation? All three comments currently in moderation on your site match terms that you’ve added to the Always Moderate list in your comment settings, which is why they are not auto-approved.

As we explained to you in that other thread, the name under which Cintra is commenting on your site does not match the display name that is set in their account. In other words, when Cintra comments on your site, they are logged out. They write in an email address connected to a WordPress.com account, but the name they use does not match the name in the settings for that WordPress.com account, so we suspect our system might be holding back those comments in case it’s someone impersonating the actual WordPress.com user.
We asked you in the other thread to ask Cintra to update their public display name, but I see they are still using a different name. Did you ask them to make this change? If not, can you ask them to please do that, and then comment on your site so we can check if this still happens?
The next time Cintra comments and it goes into moderation, please do not approve the comment, but let me know so I can check.

I can´t start to ask to everyone to change their option for user name or mode that they access the blog
Once approved once it shouldn´t hold again… people change their way to access from place to place
Suggestion: create a white list to free this one from hold…
Cintra is just one example, there are many others and again… I can´t ask everyone to change the way they access the blog

I’m not asking you to have everyone change the way they access the blog. I’m asking you to ask one user to temporarily make a change so we can see if that makes any difference, as that can help us narrow down the cause of this.
From my own testing I found the following:
1) If someone is logged in, have had a previously approved comment, and their comment does not match anything in your moderation list, the comment should go through.
2) If someone is logged out and comments using an email connected to a WordPress.com account, they’ll be prompted to log in and the comment won’t be moderated, no matter what name they entered on the comment form.
3) If someone is logged out and don’t include an email address when they comment, their comments will go into moderation every time, as we have no way of knowing if it’s the same person that was approved a previous time.
A comments whitelist won’t work in this last scenario, as that would mean ANYONE will be able to comment on your site without requiring approval if they just happen to guess the correct name that’s added to your site’s whitelist.
In order to troubleshoot this any further we’ll need to know exactly how the people whose comments are landing in the moderation queue are creating those comments to begin with.
