Oh wait, it IS pending. Okay. Well, we’ve duplicated the issue now so that’s a great start.
I need to do some testing to see if this happens on a WordPress.org installation without Jetpack. If it does I’ll file a bug report there. If it only happens on WordPress.com / Jetpack sites, I’ll file a report accordingly.

Thanks for slogging through this: I am having this exact problem on my sites and it has only started happening recently (last few months).
Previously approved commenters are now suddenly having their comments being held for moderation.
I don’t use a blacklist. There may or may not be links in their comments (everyone is approved for 2 links). They are NOT logged in.
We are a dot org jetpack shop, definitely.

It doesn’t seem like this issue has been resolved yet; I just had to fish some comments out of moderation (from long-term commenters who always get approved…so their comments really shouldn’t keep getting stuck in moderation)

I am having this problem, actually, on a dot org install. But it is interesting that you are having it on a dot com install and that it has been happening since March 2017. A third user (an other dot org user) is reporting the same problem on the dot org forum, as of 2 months and 2 weeks ago.
A “Participant” responded almost immediately, but was unable to resolve the issue, so it is still outstanding on both forums. This forum, at least, got Staff response.
I wonder if it was a JetPack upgrade.
I wonder if a bug report has been filed.
I will, of course, report back here if the dot org forum comes up with any interesting suggestions. Please be sure and do same if you manage to get the issue resolved.

FYI: The other dot org user does NOT have Jetpack installed.
Also, she listed all her plugins and the only one we have in common is Akismet.

This is very interesting…thanks for sharing…hoping that@supernovia can work with this.
My understanding was that akismet should only affect what gets put in spam, but not what gets put in moderation. Perhaps the WP team and Akismet team should evaluate if Akismet actually can affect more than just what gets flagged as spam?

Mysteriously, it doesn’t happen to all users. Right? Does it happen to only some of your users?

That’s right; it only happens to a few (I think we’ve narrowed it to 3 or 4 people on my blog), so that’s consistent.

Just for the record, I just ran a test with Akismet disabled and it didn’t make any difference.

Thanks for your patience, folks. I was able to duplicate this once, here, but cannot reproduce it on a standalone WordPress.org installation, so I’m still looking for more clues.
One thing I did notice: if I change the name field once, the next comment will be moderated. If I change it again, ditto. But as soon as I’ve approved one of those, it works fine.
Are you able to confirm the same issue there?

For the commenters who are affected on my site (J and FP), all comments go to moderation. We have experimented with having them change their name back to the “default”, comment, then we approve, then they comment again, but each comment goes to moderation. We have also experimented with having them comment without any URLs (including their own blog), approving them, then having them comment again, but it still goes to moderation.

Yes, I can confirm this definitely fits both FP and J. They already have names that they typically go by, so after the experiment, they went back to their typical names, and we’ve been manually approving every comment ever since, but they all get stuck in moderation.

Why would WordPress require that a user always use the same name? As long as they are using the same email address, comments should go through unhindered once cleared after the initial post. No?
What would be the logic behind adding additional IF statement(s)?
Is it possible that the initial name change triggered a script failure, that is then unable to sufficiently reset itself? This would be a change in the code made sometime in early Spring – or we would have all been having this problem a long time ago.
Somewhere a variable is being set inappropriately based on an inappropriately vigilant IF ELSE – and it’s not being reset.
What do you mean by “standalone” WordPress install? Is that a dot org install on a localhost?

I am a dot org user experiencing the same problem with one of my commenters (who also happens to be a site member): her comments are always held in moderation no matter what.
Re the name change theory: I went back to this person’s older comments and she did use a different name before– previously, she was commenting under her first name only. Now, she comments under her first and last name. However, I don’t remember whether her comments have always been held in moderation, or whether this only started happening after the name change… Do you know if there is a way to see which comments were held in moderation?

Two possible ways: If you get email notifications for new comments needing to be approved, then the quickest way to check would be to see if there are any “Please moderate:” emails. (This was how I was able to determine the issue occurred after a specific date.)
However, inside wordpress, you can cross-check by looking at a particular comment from the WordPress Admin Comments, page. In particular, go to the History of the comment.
If the comment went to moderation, then when it was approved, there will be a line item saying:
“[blogger username] changed the comment status to approved.”
However, there doesn’t appear to be a way from a glance within WordPress to check or search for all comments that needed to be approved, so the emails are probably a better way to start with.
