Competition Please Help

  Hello Hi,

  i am looking for sum advice and perhaps some guidance.
  i am attempting to create a competition page to my website
  i want to be able to add a disclaimer (T&C’s) at the top followed by 10 (or so) questions the user then clicks submit and has the option to share via social media ( i want to then ask the user to add there contact details in etc to be added to my mailing list) and to also select 1 winner

  i have been looking at loads and loads and loads of plugins however i have had little look the few i have tried which didnt cover all the basis are
