Hi,
I created a page contact, using the contact form. The problem is, when i publish it, the appearance is all irregular and it is already filed with my own information, i.e., whenever I visit the page, the form shows my personal information in the fields.
The blog I need help with is escambo.com

Why are you creating a contact form at all?
They are for private communication that does not appear on the blog.
All WordPress blogs by default have comments open on all Pages and Posts. You can enable and disable comments on free hosted WordPress.com blogs on this page > Settings > DiscussionDiscussion settings is where you set up defaults for comments.

Yes, there will be autofilled information in a contact form or a comment box but your information is visible to you when you are logged in and have not cleared your browser cache and cookies. No one else can see it. When someone makes a comment on your blog, unless they have cleared their cookies, WordPress.com will recognize them and complete the details for them. No one else will be able to view those details in comment boxes or in contact forms. The next person who comments will see blank fields. Commenters will not see each others details. -

If you don’t have a username account at WordPress.ORG click http://wordpress.org/support/ and register one on the top right hand corner of the page that opens, so you can post to the support forums there and receive advice from WordPress.ORG bloggers.

But…, just to make sure, when I log in “my blogs” area, the url is wordpress.com/my-blogs. Even so, I should go the support forum of wordpress.org?

No http://escambo.co/ is hosted by WordPress.com. So what I posted above does apply to your blog.
As there are comment boxes by default on all posts and pages you can simply delete the contact form you added. See what hafizr said above.
